Clinical Support

Our team provides personalised NDIS support, including daily personal and domestic assistance, community participation, capacity building, and therapeutic support. Our clinical team can assist you manage complex medical issues, mental health treatment plans, chronic disease processes, medication auditing and administration and review care plans. We work alongside participants and their treating medical team, to help manage their health better to be well enough to live comfortably and independently at home. We understand that going to hospital and medical appointments can be traumatising to some participants and recognise that early intervention and thorough health assessments can prevent unnecessary hospital admissions, which lead to better outcomes for our clients long term.
